Unfortunately it's not from a game or a manga :/

It's just concept art by Kimihiko Fujisaka for Mistwalker, it was never supposed to be more than that as far as I know (here's the complete art: http://i.kinja-img.com/gawker-media/image/upload/s--7gNb2tM8--/18hgt6kn22t4yjpg.jpg)

I think he used a similar design for a character from Terra Battle, but I guess that's about it: http://terrabattle.wikia.com/wiki/Olber
